Notify us of a change of name
If you wish to change the name listed on your Cheshire account, please send the following documents to our Regional Office address so we can process your request:
- A completed
- Your Cheshire Passbook
- 2 acceptable forms of identification. For a list of acceptable documentation, please see our
- Document/Instrument for proof of name change (i.e. deed poll, marriage cert etc)
- Written Instructions (i.e. letter)
When you have downloaded and completed your necessary form, please send it along with the items requested above to:
